Log noting for each admission, 1860-1898: name, date, county, case number, and number male or female that year. For discharges: same, plus to whom discharged and condition when discharged. For admissions, 1927-1953: name, date, time, by whom committed, and whether voluntary or not. For discharges: name, date, time, and to whom discharged.
