Scope and Content Note

Applications typically include the name and address of the applicant, age and place of birth, examination grades, educational background, and date filed as well as other information. Most applications (from circa 1928 on) also include a photo of the applicant. The earliest applications found here are spotty and some applications are missing throughout. Essential information for each licensee is entered into the department's computerized license history file (see RDA 165/0028). Information on those not renewed after 1970 will not be entered into the computer system.
