(27 archives boxes, 8 flat boxes, and 21 volumes) : Records include histories of the institution, organization charts, information on maintenance and management of the physical facilities, grant administration records, newsletters and correspondence of parent organizations, population movement records, superintendent's speeches, reports, staff records and other subject files. Population records include admission books detailing each patient's family background, physical description, and medical history; census reports of patients and staff; a record of admissions by date or county; and other records. Reports include activity reports to the Division of Mental Hygiene containing narrative and statistical data; studies and surveys of services by staff and others; and investigation reports. Staff records pertain to in-house workshops and employee procedure and orientation manuals. Other files concern work placement of patients and sterilization. : The volumes and scrapbooks in this series have been prepared for microfilming; care should be taken to preserve the original order.

This accession includes a small number of personal photos of Anna Lenz, Lenz lived at the NWCDD from 1923-1990. Lenz was not committed to the home for a disability, rather as a punishment for an accident and served as a caretaker for Cottage Number 1 and several children who lived at the Center. Accession contains materials originally accessioned as 1991/134, 2000/011 and 2009/004. : Reproductions of images of patients requires permission of the Visual Materials Curator or the Visual Materials Archivist.
